Tom cat urine smells the way it does because of their relatively high levels of testosterone compared to neutered male cats.

There are three possible sources of levels of testosterone high enough to produce urine odor:

1. Testicles. A cat may appear to have been neutered, but if the urine has tom cat odor he may have had retained testicle(s) that were not removed. If examination of the cat's penis reveals spines on the surface then there is most likely a retained testicle. 

2. A tumor in the adrenal gland. This is a rare problem in cats.

3. Administration of testosterone as a drug.  This can occur accidentally, for example, if the cats owner is being treated with testosterone cream that the cat is contacting.

Once a cat is neutered the testosterone levels decline significantly within 24 to 48 hours. Allowing for some variation in how quickly this occurs, and traces of tom cat urine that may be "clinging" to the cat's urinary tract, tom cat urine odor should be barely detectable or gone within a week after neutering.
