Please review the following information prior to your pet’s visit with us. You can ask questions and let us know your preferences for optional services and additional procedures at your convenience, before the appointment by phone or email, or at the time or admission.

Nutrition What does your cat eat, how much, and what is the feeding schedule?
Lifestyle Indoor only, indoor/outdoor, or outdoor only?
Vaccines For cats one year and older, we recommend a yearly rabies vaccine and a FVRCCP (distemper, upper respiratory) every 3 years. For cats that go outdoors we recommend a leukemia virus vaccine yearly.
Internal parasites If your cat goes outdoors, or is indoor-only but catches mice, we recommend giving a broad-spectrum dewormer once or twice a year.
Fleas and ticks Are you using flea and tick control? Do you need a refill? If so, what brand and how many would you like?
Dental care We recommend our professional cleaning and evaluation yearly for dogs 1 year or older. Would you like to schedule this, or have us provide you with a reminder for it at some future time?
Dental care at home Daily brushing is the gold standard, and we can provide the brush, toothpaste, and instructions. We also stock less labor-intensive options, including dental chews, drinking water supplements, and diets that fight plaque and tartar build-up.
