Please review the following information prior to your pet’s visit with us. You can ask questions and let us know your preferences for optional services and additional procedures at your convenience, before the appointment by phone or email, or at the time or admission.
Our check-ins contain our best, most thorough general recommendations. We will help you edit them to create a health care plan that is what you want for your pet.

Nutrition What are you feeding your pet, how much per meal and what is the schedule?
Activity level and lifestyle Is your dog active or sedate? Is he a working dog, a hunting dog, a show dog or a guide dog? Does he do agility training or regularly get together with friends at doggie daycare?
Vaccines For most dogs over 1 year old, we advise: DA2PPV (distemper, parvo, etc) and rabies vaccines every 3 years and lyme disease, leptospirosis and kennel cough vaccines yearly.
Heartworm test and preventive If your dog is over a year old and has not taken monthly heartworm preventive for three or more consecutive months then we advise a heartworm blood test. We recommend giving the monthly preventive medication year-round. Do you need to pick up some more preventive? If so, what brand and how many would you like?
Internal parasites other than heartworm We recommend an annual fecal test for intestinal parasites. If you would like to do this, please bring a specimen with you - a teaspoon of stool produced within a day of your appointment, not frozen and not dried out, is sufficient.
Fleas and ticks Are you using flea and tick control? Do you need a refill? If so, what brand and how many would you like?
Dental care We recommend our professional cleaning and evaluation yearly for dogs 1 year or older. Would you like to schedule this, or have us provide you with a reminder for it at some future time?
Dental care at home Daily brushing is the gold standard, and we can provide the brush, toothpaste, and instructions. We also stock less labor-intensive options, including dental chews, drinking water supplements, and diets that fight plaque and tartar build-up.
Any extras? Does your dog need a nail trim or it’s anal sacs emptied? Do you need a prescription refill, more food, or pet supplies?
