YVC Check-ins are lists of questions we use to make sure your visits with us are complete. Each check-in is dedicated to a specific reason for a visit, for example, a canine annual, feline annual, new puppy, and so forth. Some items on the check-ins are informational, such as updating your contact address and numbers, and other items are the services we will offer you. YVC Consents are the consent forms we use for various procedures.
We hope providing the check-ins in advance will make them very useful to you. Most of the check-ins and consents are forms that you can fill out here and email to us by clicking "submit". All of them allow you to review the relevant information and questions prior to your visit with us.
You can use the check-ins in a variety of ways: prepare questions and get more detailed information before you arrive; run through the check-in with one of our technicians on the phone; email us your preferences; print out your answers and bring them with you. If you do not complete the check-in prior to your office visit or bring printed answers with you then your technician will review the list when you arrive at YVC.
