YVC SURGERY CHECK-IN AND CONSENT

CHECK-IN

PRE-ANESTHETIC BLOOD PROFILE: We strongly recommend a blood profile prior to anesthesia and surgery. This profile works as an extension to our physical exam, providing us with valuable information about your pet's "inner workings". It also serves as a baseline for comparison if your pet should need future diagnostics.

ADDITIONAL DIAGNOSTICS: Diagnostics such as heartworm test, feline leukemia/aids test, urinalysis, xrays, and ultrasound may be important for assessing your pet's health status prior to surgery. We will help you sort through these options.

ADDITIONAL SERVICES: Take advantage of your pet's stay with us to have additonal services, including annual exam, vaccines, nail trim, microchipping, etc. Depending on what your pet is having for a primary procedure, you may be able to choose to take advantage of the anesthesia to have us do a dental cleaning and evaluation, remove a troublesome wart or cyst, and so forth.

ADMISSION AND DISCHARGE

ADMISSION: Your pet should be dropped off between 7:30 and 8:30 on the morning of surgery. She or he can have water free choice and a half-sized breakfast prior to coming in unless we have instructed you otherwise.

DISCHARGE: We make every effort to reunite you with your pet as soon as possible. Most pets go home from most procedures between 4 and 6 pm. When we hospitalize a pet overnight it is because that is the best way for it to begin to recover from it's procedure.

CONSENT

General Consent: I understand there are risks associated with any medical procedure, including anesthesia, surgery, treatments, diagnostics, and hospitalization, and I release YVC from all liability in the absence of gross negligence.

Estimate and Payment: I have received or waived an estimate for today's procedure, and I understand that payment in full is expected when my pet is discharged to me.

Hospitalization: I understand that YVC is staffed from 7am to 6 pm Monday through Friday and, with intermittent breaks, from 8 am to 6 pm Saturday, Sunday and holidays, and that there are no staff members routinely present at any other time.

Emergencies: I authorize emergency treatment of my pet and I accept financial responsibility for this care. I have specified any particular details of this care, such as a do-not-resuscitate order, in the space below.
